Each polo shirt costs $25.

Please submit your order for you SCBC Polo Shirt by Friday, May 10.

To make your payment, for the shirts, please via the instructions at the bottom of this form.

If you would rather pay via cash or check, please fill out this form and then place your cash or check in one of the offering boxes in the Scott Auditorium. Please be sure to put your name on your cash/check and indicate that it is for payment of polo shirt(s).

Please Note: To order multiple shirts, you will need to fill out seperate forms. But, you can make one payment for multiple shirts.

To make your payment online, go to www.southcharlottebaptist.org/give and click on the "Give Online Now" button.

When the giving form appears, if you have already created a profile with Tithely, you may sign in. Otherwise, you can either create a profile (making future giving easier) by clicking the "Sign Up" button (in the upper right corner of the form) or you can complete the form as a "Guest" by filling in all the appropriate information.

When completing the payment form, please be sure to select "Nondeductible Expense" in the drop down box to the right of the "Amount."
